abstract | FIELD: medicine. n SUBSTANCE: invention relates to medicine, namely, to ophthalmology, and can be used for surgical treatment of dipnoous idiopathic macular rupture. Method comprises administering the dye intravitreally, vitrectomy with removal of the posterior hyaloid membrane, separation of 2.0–2.5 mm to the lower temporal arcade from the foveola of the tip of the internal limiting membrane (ILM), gripping the tip of the ILM with tweezers, conducting circular muculorhexis using intravitreal tweezers, without detachment of the ILM from the edge of the rupture, placing the ILM inside the rupture, replacing the liquid with air. As a dye, a suspension of "Vitreocontrast" is used, which is injected into the macula and paramacular area first before the posterior hyaloid membrane removed and then, before the circular maccorrexis conducted, 0.5 ml per injection. Pair of pincers is used to grip an ILM end; the membrane is separated throughout 2–3 o’clock meridians by doing a motion directed along a segment of an imaginary circle with the macular rupture in the center. Intercept the arc-separated ILM (fragment ILM) at the end point and continue cutting off the ILM. Thus, by successive interceptions with tweezers, the edges of the ILM fragment produce a circular separation of the ILM around the macular rupture. Then, the edge of the ILM fragment is trimmed with the vitreotome. After laying a fragment of the ILM into the fracture, press it towards the center of the foveola and replace the liquid with air. n EFFECT: method provides reducing the number of injuries accompanying the surgical intervention, eliminating the vertical and horizontal retinal tractions, closing the macular rupture, which leads to improved visual functions, the disappearance of metamorphopsia and central scotoma. n 1 cl, 1 ex |
