abstract FIELD: medicine. n SUBSTANCE: instrument comprises a body with an in-built wave guide containing ultrasonic vibration concentrator with a needle on a working end, a piezoelectric cell assembly and a bearing sleeve in the centre of which there is a canal for eye fluid aspiration. Between the body and the concentrator there is a space which is filled with in irrigation solution. Between the piezoelectric cell assembly and the concentrator from one side and the piezoelectric cell assembly and the bearing sleeve from the other side there are insert elements in the form of truncated cylinders with one basis having an angle of skew 75-85° and the other basis inclined 90° to a central axis of the wave guide from a material having a different ultrasound conduction velocity from primary materials of the wave guide of the ultrasonic instrument in 1.5-2 times. The bases of the concentrator and the bearing sleeve have an angle of skew 75 to 85°, while the concentrator and the bearing sleeve are positioned relatively to each other rotationally about the central axis at 0 to 180° with respect to the skewed planes. n EFFECT: use of the invention reduces a degree of manifestation of a repulsive phenomenon and provides the concentration of a cavitation cloud close to an end face of a hollow needle. n 2 cl, 4 dwg, 2 ex
