abstract A treatment instrument for an endoscope has: a pre-curve part (4) provided to the distal part of a sheath (3) and having resilience toward a curved shape in which the sheath curves along a virtual plane (α) including the central axis of the sheath; a knife wire lumen (20) having a central axis in a position separated from the virtual plane in the tip part of the pre-curve part, the knife wire lumen (20) being formed along the central axis of the sheath; a guide wire accommodating part (7, 9) into which the guide wire can be inserted, the guide wire accommodating part (7, 9) being formed along the longitudinal axis of the sheath in a position separated from the knife wire lumen in the circumferential direction about the central axis of the sheath; and a distal slit part (10d) formed along the central axis of the sheath so that the guide wire accommodating part and an external peripheral surface on the outside of the curve in the pre-curve are communicated from the proximal end of the pre-curve part to an intermediate portion of the pre-curve part toward the distal side.
