titleOfInvention System and method for longitudinal disease management
abstract A longitudinal medical outcome collection and reporting (LMOCR) system provides a system for collecting, storing and displaying medical information. In particular, the system includes a patient database for storing medical records and information pertaining to a plurality of patients. The LMOCR system further includes an application server that stores applications for communicating with the patient database, a remotely located patient device, and a remotely located provider device. The application server includes a patient module that displays on the patient device a patient portal that allows the patient to interact with the LMOCR system. The patient module provides a questionnaire to the patient portal to collect information from the patient and applies a first set of rules to collected patient information to selectively modify the questionnaire to collect relevant follow-up information from the patient. The application server further includes a provider module that displays on the provider device a provider portal that allows the provider to interact with the LMOCR system. The provider module applies a second set of rules to collected patient information to translate the collected patient information into a format relevant to the provider, wherein the provider is able to review, modify, and add to the collection of patient data stored in the patient database.
