abstract Methods and systems for transosseous access and repair of joint surface are disclosed wherein one embodiment of the method comprises inserting a reaming rod through a bone tunnel positioned perpendicular to a first joint surface, connecting a reaming blade to the reaming rod, reaming the first joint surface with the reaming blade and removing the reaming blade and the reaming rod. In some embodiments, the reaming blade is configured to be inserted into the joint through an arthroscopic access portal. In some embodiments, an implant system comprises an implant anchor and an implant surface cap and the implant anchor is adapted to couple with a fixation element to anchor the implant anchor in a bone.
