abstract A method, system, and computer-readable medium to stratify a health risk acquires structured information and unstructured information from a plurality of sources, converts at least some of the unstructured information to structured information, and geocodes the acquired structured information and converted structured information to produce geocoded data. Health information of a plurality of subjects are similarly geocoded to produce geocoded subject health data and merged with the geocoded data and mapped to a dashboard. First geographical correlations between at least some of the health information and the structured and unstructured information derived from the merged geocoded data and geocoded subject health data are output for display, and a risk stratification base score is developed in accordance with second geographical correlations between at least some of the health information and the acquired structured and unstructured information derived from the merged geocoded data and geocoded subject health data. The risk stratification base score is adjusted in accordance with positive and/or negative variables, and a total risk score is determined from the adjusted risk stratification base score and output for integration into an electronic health records system.
