abstract | Method of plastic closure of marginal defect of upper sagittal sinus (BCS) after total removal (gradation I according to Simpson classification) of parasagittal meningioma (PM), which causes BCC obstruction (gradation VI according to Bonnel and Brotchi classification), while preserving collateral blood flow pathways surgical treatment of parasagittal meningiomas. When accessing the parasagittal meningioma, the incision is marked on the skin, then soft tissue is prepared along and around the incision line, and thus a periosteal flap is prepared for separation and mobilization because, when impregnated with fluid, it will be enlarged and more elastic. Saline solution or its mixture with adrenaline and local anesthetic (lidocaine) is used for hydropreparation. After incision of the skin, subcutaneous tissue, aponeurosis and after hemostasis, perform separation and mobilization of the oxidative flap, always maintaining the contact area (feeding leg), at least 6 cm wide, located parallel to the sinus on the opposite side from the main node of the tumor. A sterile napkin soaked in saline is placed on the prepared periosteal flap and all planned stages of tumor access and removal are performed sequentially. During the total removal of parasagittal meningioma (gradation I according to Simpson's classification), which sprouted all the walls of the BCC and caused its obstruction (VI according to the Bonnel and Brotchi classification), while maintaining sufficient collateral blood flow, perform radical removal of the BCC involved in the pathological process. The edge defects of VSS which have arisen thus demand plastic closing. |
