abstract FIELD: medicine.SUBSTANCE: invention relates to medicine, namely to anesthesiology, and can be used for combined prolonged high spinal and endotracheal anesthesia during operations on the upper floor of the abdominal cavity. The method includes performing high spinal anesthesia on the patient in the side lying position. After local anesthesia, median access of puncture and catheterization of the spinal space at the level of L4-L5 using a set for prolonged spinal anesthesia, in which the catheter is advanced in the cranial direction by 3 cm with the introduction of an isobaric solution of marcaine into it at an initial dose of 20 mg with further prolongation of the introduction drug and regulation of blockade control by tilting the head end of the table up to 45°. After the administration of fentanyl 2 mcg/kg and the muscle relaxant trakrium 0.5 mg/kg and inhalation anesthesia with sevoflurane 1.5% with tracheal intubation, ketamine is administered once intravenously at a dose of 100 mg.EFFECT: method provides complete segmental blockade and muscle relaxation, stability of hemodynamic parameters, as well as reduction of doses of narcotic analgesics due to a combination of two types of anesthesia and additional administration of ketamine.1 cl, 1 ex
