abstract | FIELD: medicine.SUBSTANCE: invention relates to medicine, namely to anesthesiology and resuscitation, and can be used for perioperative pain relief in operations on the lower extremities. Paravertebral blockade is performed under the control of neurostimulation so that the needle is inserted at a distance of 3-4 cm from the spinous process of the L3 vertebra with the direction of the needle cut cranially perpendicular to the skin in the sagittal and transverse planes. The needle is advanced to contact with the transverse process, after which it is pulled back to a depth of 1-2 cm and inserted at an angle of 10-15° cranially, 1-2 cm deeper than contact with the transverse process in the transverse plane until contractions of the internal oblique abdominal muscle appear. Next, a posterior approach to the sciatic nerve is performed with the introduction of lidocaine 1% 100 mg and ropivacaine 0.375% 50 mg.
