abstract FIELD: medicine.SUBSTANCE: invention relates to medicine, namely to phlebology. Vascular isolation of the inferior vena cava (IVC) and all its tributaries is performed. In this case, the upper boundary of the IVC isolation is made above the head of the floating thrombus to prevent fragmentation of the thrombus. A longitudinal venotomy 6-8 cm long is performed directly next to or with the transition to the source vein of the floating thrombus, namely the common iliac, ovarian vein. Then thrombectomy of a floating thrombus is performed. The entire isolated area of ​​the IVC and the mouth of the vein-source of the floating caval thrombus are freed from blood clots. The cavatomic opening is sutured with an atraumatic thread. The handles are removed from all localizations, with the exception of the holder on the source vein of the floating thrombus. Next, plication of the source vein of the floating caval thrombus, namely the common iliac or ovarian vein, is performed by applying 1-2 sutures in the transverse direction along the vessel, dividing a single lumen of the vein into 2-3 channels. At the end of the surgical intervention, drainage of the retroperitoneal space and abdominal cavity and suturing of the surgical wound are performed.EFFECT: method allows one-stage prophylaxis of pulmonary embolism by removing the floating head of a thrombus, excluding early and late complications associated with the occurrence of inferior vena cava syndrome, as well as maintaining intensive blood flow through the IVC released from thrombi and creating a barrier to the spread of thrombotic masses into the IVC.1 cl, 2 ex, 6 dwg
