abstract FIELD: medicine. n SUBSTANCE: invention refers to medicine, namely to traumatology and orthopedics, and can be used for surgical treatment of pyoinflammatory processes of bone and soft-tissue structures of the patient's locomotor system. In preoperative period before surgical treatment of pyoinflammatory processes of bone and soft-tissue structures of patient's locomotor system is determined by standard X-ray, fistulography and ultrasonic diagnosis of the volume and boundaries of a pyoinflammatory process in the region of the locomotor system of the patient. Area of pyoinflammatory complications is accessed, and necrotic bone tissues are excised to a support cortical layer with simultaneous removal of pathologically changed soft tissues. Flow-aspiration drainage of the formed cavity is performed by antiseptic solutions cleaned from pathologically changed bone and soft-tissue structures. Volume of the postoperative defect bone cavity is filled with a pre-prepared spacer in the form of a macroporous cryogel based on polyvinyl alcohol impregnated as an antibacterial agent with aqueous solutions of an aminoglycoside and / or glycopeptide with concentration of 10–50 mg/ml, wherein aminoglycoside antibacterial aqueous solution of spacer used is an aqueous solution of gentamycin, and as the glycopeptide of the antibacterial aqueous solution of the spacer use an aqueous solution of vancomycin, the wound is closed in layers, the spacer is removed in 40–60 days from the bone cavity of the postresection defect. n EFFECT: method provides preventing pyoinflammatory complications, reliable restoration of anatomical and functional integrity, as well as motor capabilities by using a spacer impregnated with antibacterial solutions. n 5 cl, 5 ex
