abstract FIELD: medical equipment. n SUBSTANCE: group of inventions refers to surgical instruments, particularly, to tools for cutting and suturing. Surgical instrument comprises handle, elongated journal, working tool, working element, launching trigger and system of dissection. Elongated journal is functionally connected to handle and protrudes from it. Working tool has proximal end functionally connected with elongated journal, wherein working tool has first branch made with possibility of selective movement relative to second branch under action of start-up motions transmitted to it. First branch can be functionally hold staple cartridge. Working element is supported to displace relative to elongated journal and is made with possibility of selective message start-up motions of second branch of working tool for movement of second branch from open position to closed position. At that, closed positions, at least, contain maximum fixing position, in which second branch is located relative to upper surface of cartridge with clips to provide for fixation of tissue in between brackets in cartridge with clips and completely activated position, in which brackets are completely formed. Launching trigger is supported by handle assembly and functionally interacts with working element, forcing working element to communicate start movements to second branch. Incision system is made with possibility of selective actuation and comprises scalpel and trigger mechanism. Scalpel is supported to displace relative to elongated journal and is made with possibility of movement of inactivated position near proximal end of working tool in activated position on distal end of working tool under action of cutting movement communicated to it, which is independent from starting motion. Trigger mechanism of scalpel movement is supported by handle assembly and functionally interacts with scalpel, communication cutting movement to it. Surgical instrument in second version comprises first branch, second branch and scalpel. First branch is made with possibility of functional support of implanted cartridge with clips. Second branch is supported with possibility of displacement relative to first branch and has possibility of selective movement from open position, in which second branch is located at distance from implanted cartridge with clips in first branch, into closed position, in which second branch compresses implanted cartridge with clips between first and second branches under action of selective communication of starting movement to second branch by means of launching system interacting with it. At that, closed positions, at least, contain maximum fixing position, in which second branch is located relative to upper surface of cartridge with clips to provide for fixation of tissue in between brackets in cartridge with clips and completely activated position, in which brackets are completely formed. Scalpel is functionally maintained relative to first and second branches and is made with possibility of selective movement from inactivated position at first end of first branch into activated position at second end of first branch under action of cutting movement communicated to it, which is independent from starting motion. n EFFECT: inventions provide both tissues cutting during application of brackets, and possibility of fasteners installation by surgeon without dissection of tissue. n 14 cl, 243 dwg
