titleOfInvention Method for removing heavy silicon oil from vitreal cavity
abstract FIELD: medicine.SUBSTANCE: irrigation and troacar aspiration cannulas are inserted in a flattened portion of the ciliary body through microapproaches. The troacar aspiration cannula is short. The aspiration cannula is immersed into the silicone oil medium and a distal end of the cannula is preserved inside the oil pocket throughout the whole stage of oil discharge, including removing the last drop. The oil aspiration starts in vacuum in an aspiration system of 600 mmHg. That is combined with irrigating balanced saline under pressure 21-25 mmHg. The oil is removed partially with leaving the distal end of the aspiration cannula covered with an oil layer. The irrigation is reduced to 15-20 mmHg. That is combined with scleral buckle to the centre of the vitreal cavity and upwards by means of an instrument delivered behind the equator from the opposite side from an insertion point of the irrigation cannula. The scleral buckle continues until the oil is aspirated completely. The silicone oil keeps up to be aspirated. The vacuum is reduced in the aspiration system to 200-400 mmHg, when the distal end of the cannula is surrounded by the last drop of the silicon oil. The oil drop is removed. The distal end of the cannula is held tightly in the middle of the drop.EFFECT: method provides fast, reliable and injury-free complete removal of the silicone oil from the vitreal cavity.2 cl, 2 ex
