titleOfInvention Method for laser treatment of pathological lachrymal passages
abstract FIELD: medicine. n SUBSTANCE: invention refers to medicine, namely to ophthalmology, and can be applicable for laser treatment of lachrymal duct obstruction (LDO). An opening of the lachrymal duct is dilated. A hollow conductor with a laser light guide is inserted into an outer one-third of the lachrymal duct. The laser light guide contains two types of emission - Ng:Yad laser and low-power helium-neon laser. The low-power helium-neon laser is actuated, and the conductor is advanced with the actuated low-power helium-neon laser along the lachrymal duct lumen to the lachrymal sac until it stops against the bone. The Nd:YAG laser is actuated at a wavelength of 1.44 mcm. A trepanation opening of the diameter of 5-8 mm is created in the bone within the lachrymal sac junction into the nasolachrymal canal at pulse energy of 150-280 mJ, pulse frequency 25-30 Hz, pulse duration 50-80 mcs. The bone exposure length makes 0.6-3 min. The nasal mucosa is dissected away around the trepanation opening with the diameter exceeding the diameter of the trepanation opening by 2 mm. The Nd:YAG is de-actuated, and the low-power helium-neon laser keeps emitting for 30-40 sec. The working laser light guide generating the low-power helium-neon laser is slowly removed with its exposure covering the LDO mucosa. An intubation material is placed into the lumen of the formed lachrymal passages. n EFFECT: method enables performing surgical intervention combined with a phototherapeuic exposure, reducing the number of injuries and a postoperative inflammatory reaction by a combined effect of two types of the laser exposure. n 2 cl, 3 ex
