titleOfInvention Method of preventing wound complications after prosthetic hernioplasty in case of infringement of ventral hernias
abstract FIELD: medicine. ^ SUBSTANCE: invention relates to medicine, namely to surgery, and can be used in carrying out prosthetic hernioplasty in case of infringement of ventral hernias. For this purpose installation of endoprosthesis is performed and antibiotic prevention is carried out. On both sides from access in projection of the mesh endoprosthesis edge separate punctures of skin, located from angle of mesh prosthesis at the distance equal 1/3 of side length, are made. Into punctures parallel to access into periprosthetic tissues introduced are polychlorbinyl tubes with 0.9-1.0 mm diameter and holes with diameter 0.3 mm, located on lateral surface of tube every 20 mm. Length of tubes corresponds to length of mesh endoprosthesis. Tubes are placed above the plane of mesh endoprosthesis. On the first and third day of post-operative period 400 ml of patient's blood are sampled into "Hemacon" system. Obtained blood is separated by centrifuging in mode 3700 revolutions per minute during 20 minutes into plasma, leuko-platelet and erythrocyte masses. Erythrocyte mass and plasma are returned to patient, into leuko-platelet mass 200 ml of 0.9% NaCl solution are added, centrifuging is repeated in the same mode with removal of supernatant layer. Leuko-platelet mass is cultured with 500000 IU of recombinant interleukin-2 and 200 ml of 0.9% NaCl solution during 60 minutes at temperature 37C with further centrifuging in the same mode with separation of supernatant liquid, which contains complex of autologic cytokines. Obtained complex is introduced into polychlorvinyl tubes in dose 1 ml per each cm2 of mesh endoprosthesis area on the 1 and 3 day after the operation. Leuko-platelet mass is returned to patient. ^ EFFECT: method ensures prevention of development of wound complications and acceleration of healing process in area of mesh endoprosthesis due to impact on primary-aseptic inflammatory reaction in periprosthetic tissues. ^ 2 cl, 3 dwg, 2 ex
