abstract | FIELD: medicine. n SUBSTANCE: invention relates to medicine, in particular to cardiology, and deals with treatment of acute left ventricular insufficiency (LVI) of I-III class in patients with diabetes mellitus. For this purpose complex treatment, which includes oxygen-therapy 6-8 litres per minute, lifted bed head, introduction of morphine hydrochloride intermittently 1% - to 1 ml intravenously, nitrates, diuretics, inotropes, metoprolol in dose 6.25 mg 2 times per day and ivabradine. If AP is not higher than 100 mm Hg and HR is 100-110 per minute, ivabradine is introduced in day dose 10 mg in two steps, if HR is 120-125, ivabradine is introduced in day dose 15 mg in two steps under control of heart rhythm and conductivity and with AP measurement every 6 hours. n EFFECT: method makes it possible to efficiently reduce sinus tachycardia, which disrupts pathologic vicious circle, connected with shortening of heart diastole and improves heart blood supply and left ventricular contractility, which, in its turn, ensures more efficient relief of I-III class LVI symptoms, reduction of treatment terms and reduction of lethal outcomes in said group of patients. n 2 tbl |
