abstract | FIELD: medicine, surgical urology. n SUBSTANCE: one should sample vascular transplant, prepare a recipient site, create the access towards internal epigastric vessels in area of median border of the rectus muscle of abdomen and great subcutaneous vein in area of the upper femoral third, transfer neophallus body to the recipient site to fix and anastomose its vessels with lower epigastric vessels and great subcutaneous vein. As a vascular transplant one should apply radial vascularized fragment upon radial vessels. It is important to form neophallus upon plastic phalloprostheses. Neophallus consists of neophallus body and neourethra. One should mobilize penile skin in area of its root, vascular-nervous bundle and urethra, intersect cavernous bodies, prepare recipient site at temporary keeping the circulation in area of distal penile department, create the access towards internal epigastric vessels in area of median border of the rectus muscle of abdomen, to the great subcutaneous vein in area of the upper femoral third, transfer neophallus onto recipient site to fix it. It is necessary to fix phalloprostheses in cavernous bodies of penile pedicles, apply urethro-neourethroanastomosis; radial vessels should be anastomosed with lower epigastric vessels and the great subcutaneous vein, transfer a distal penile fragment onto neophallus' distal end, fix cavernous bodies on phalloprostheses, anastomose dorsal penile vessels with radial artery and vein, apply neourethro-urethroanastomosis. The innovation decreases traumatism of surgical interference, excludes the procedure of urethroplasty as the second stage to reconstruct "male-type" urination, spheroidal contraction of neophallus and nonconformity between the transplant and the neophallus under formation. n EFFECT: higher efficiency. n 1 ex |
