abstract 1. Inhaler for an inhalation preparation, comprising a container containing a pharmaceutical composition under pressure; a metering valve for dispensing a measured dose of the composition from the container for administration to the patient, if necessary; and a mouthpiece for releasing a metered dose to a patient; where the mouthpiece includes an outlet having a diameter in the range from 0.2 to 0.4 mm; and wherein the pharmaceutical composition comprises an anticholinergic and a pharmaceutically acceptable propellant. The inhaler according to claim 1, wherein the anticholinergic agent is selected from tiotropium, ipratropium, oxytropium, aclidinium, their pharmaceutically acceptable salts or solvates, or a mixture of the above substances. The inhaler according to claim 1 or 2, wherein the anticholinergic agent is tiotropium bromide. The inhaler according to claim 1, wherein the metering valve is designed to spray a metered dose of a pharmaceutical composition containing from 2.5 to 18 micrograms of the agent or each of the cholinergic agents. The inhaler according to claim 1, wherein the propellant comprises one or more pharmaceutically acceptable HFC propellants and / or one or more pharmaceutically acceptable hydrocarbon propellants. The inhaler according to claim 1, wherein the composition further comprises a co-solvent, which preferably is a polar co-solvent.
