Diseases such as headaches can be treated by administration of botulinum toxin to a patient suffering from such a disease, for example migraine headaches. Conjoined fixed location / fixed doses and options follow the pain variable dose, and an injection location paradigm is disclosed to optimize the clinical effectiveness of botulinum toxin administration in patients with chronic migraine headaches. The injection schedule is a fixed-position, fixed-dose injection regimen within 31-39 locations throughout the seven head / neck muscles, namely, the frontal muscles, the pectoral muscles, the nasal muscles, the larynx, the temporal muscles, the mitral muscles and the cervical spinal lateral muscles.
