abstract The present invention relates to an integrated structural unit comprising a diagnostic form comprising one or more active ingredients advantageously in an amount that does not change by more than about 5% from a predetermined target amount. In one embodiment, the unit form comprises a substrate, an attachment located on the substrate and a spreading layer that holds and spreads a liquid sample present on and assayed on the attachment. Attachments include powders comprising the active ingredient (s). Diagnostic forms are made through a dry powder attachment device that electrostatically attaches the powder onto a substrate using an electrostatic chuck and a charged powder delivery device.
