titleOfInvention | Method for identifying whether a patient with acute decompensated heart failure (ADHF) exhibits a hypercoagulable state |
abstract | Subjects with heart failure (HF) are at higher risk of developing thrombosis. Therefore, we determined the ability to produce thrombin in patients with acute decompensated heart failure (ADHF). Those prospective trials included 34 acute decompensated heart failure patients and 30 control inpatients without heart failure. Endogenous thrombin generating capacity (ETP) using platelet rich plasma was higher at admission in patients with acute decompensated heart failure and remained elevated during admission compared to controls. Soluble markers of endothelial dysfunction, including von Willebrand factor, Factor VIII, and endothelial protein C receptor, were higher at admission in patients with acute decompensated heart failure. Plasma levels of DNA-histone complex were significantly elevated at admission in patients with acute decompensated heart failure compared to controls. Higher concentrations of Annexin V/Tissue Factor (TF) positive endothelium-derived extracellular vesicles were also found on admission in patients with acute decompensated heart failure compared to controls. The finding is that a method for determining thrombin generation in patients with acute decompensated heart failure (ADHF), which combines measurements of endogenous thrombin production and endothelial dysfunction, stratifies patients and therefore treatment. Demonstrate particular value for orientation and patient follow-up. |
