abstract | PROBLEM TO BE SOLVED: To provide an application form that can be applied for a plurality of different purposes by a simple operation. The present invention is divided into three regions (a, b, c) connected via a planned folding line ((X) or (Y)) that also serves as a planned cutting line, and the three regions (a , B, c), a peelable adhesive structure 20 is formed in the central region b, and two selected regions ((a, b) or (b, c)) among the regions are folded. A copy structure 40 is formed between the first paper 30 that is folded in half along a planned line and the second paper 60, and is opposite to the surface of the second paper 60 on which the copy structure 40 is formed. A linear adhesive structure 10 is formed on at least a part of the inner side of at least one of the four sides of the surface, and a third paper 90 that overlaps the second paper 60 is formed via the linear adhesive structure 10.
