abstract An embolectomy catheter, rapid exchange microcatheter, system and method for removing blood clots or obstructions from blood vessels are provided. An embolectomy catheter can be advanced with or over a guide wire 878 that has been previously inserted through or near the clot 872, A clot removal device is provided that can be deployed from the catheter after it has been advanced at least partially through the clot. The clot removal device comprises a wire nest that is designed to prevent passage of the clot and can be spread out, the delivery catheter comprises a telescopic inner tube and an outer tube, and the clot removal device is provided by the outer tube. Constrained in the radial direction. When the outer tube is retracted and the clot removal device is removed from the clot removal device, the clot removal device expands into an expanded configuration. The infusion guidewire is useful in combination with an embolectomy catheter and allows the infusion of drug or visualization fluid from the clot distal to the clot. [Selection] Figure 24B
