abstract An apparatus and method are provided for placing and advancing diagnostic or therapeutic devices in a hollow body organ of a serpentine or unsupported anatomical structure. A distal region having a handle, an overtube disposed within a hydrophobic sheath, and an atraumatic tip. The overtube can be removable from the handle and has a longitudinal axis at an angle from the handle. The sheath can be disposable to allow reuse of the overtube. A fail-safe tensioning mechanism is provided that selectively hardens the overtube to reduce organ swelling caused by advancement of the diagnostic or therapeutic device. This fail-safe tensioning mechanism reduces the risk of overtube reconfiguration when the tensioning system is defective. [Selection] Figure 2
