An ultrasonic tissue ablation device is disclosed comprising a laterally vibrating small diameter probe (10) and a coupling assembly for probe attachment and detachment that allows the probe (10) to be separated from the device body. This probe detachment enables the probe (10) to be inserted, operated, and removed independently of the device body. This probe (10) may be used with an acoustic sheath and / or a suction sheath to enhance tissue ablation. The device body comprises an ultrasonic energy source and a horn assembly (34).
