The extension shaft has a first curved portion near a distal end of the extension shaft. The first curved portion of the extension shaft forms a first plane. The extension axis has a second curve disposed between a distal end of the extension axis and a first curve of the extension axis, the second curve forming a second plane. The second plane intersects the first plane at a fixed angle. |
