abstract (57) [Summary] [Problem] A sheet-shaped information display / recording medium capable of recording a large amount of information at a low price, suitable for mass production, made of a material which does not cause environmental problems even when disposed, and a sheet thereof. Provided is an information reading device for reading recorded information on a state information display / recording medium. SOLUTION: A notification postcard 1A as a sheet-form information display / recording medium has a visual information display section 10A in which information which can be visually observed is described, and an optically readable information recording section 11A. The optically readable information recording unit 11A encodes information related to the contents described in the visual information display unit 10A, and if necessary, adds an error correction code and encrypts the information. Information encoded in a carla code or the like is printed. Although the symbols printed on the information recording unit 11A are visible, they are encoded and encrypted, so that the contents cannot be read by a bar code reader of a POS terminal. For the interpretation, a floodbed scanner or the like is used.
