An anti-fungal and anti-bacterial composition comprises nystatin and an alkyl isoquino- .linium salt of the formula: where R is an alkyl group of from 8 to 18 carbon atoms or a mixture thereof and X- is a pharmaceutically acceptable anion e.g. Cl-, Br-, ยข[SO 4 <SP>2-</SP>]. The compositions may be applied topically in the form of non-aqueous based ointments, lotions comprising aqueous suspensions of pH 4 to 6 and creams which may contain hydrophobic material. The compositions may also include anti-inflammatory steroids e.g. hydrocortisone and prednisolone, skin softeners such as isopropyl myristate and conventional excipients. In a specific example, R is a mixture of alkyl groups in the proportions by weight of 50% C 12 , 30% C 14 , 17% C 16 and 3% C 8- C 18 and X- is Br-.
