abstract A process to prepare 3-cephem-4-carboxylic acid compounds disubstituted in positions 3 and 7 of formula-01: ** (Formula-01) ** where R1 is amino or a protected amino, R2 is a hydrocarbon group aliphatic, which may have a suitable substituent or substituents, R3 is carboxy or a protected carboxy, and R4 is a 5-membered heteromonocyclic group, containing N, having one or more lower alkenyl groups, provided that R2 is not methyl when R4 is tetrazolyl having a lower alkenyl group, or its pharmaceutically acceptable salts, which process comprises reacting a compound of the formula-02: ** (Formula-02) ** where R3 and R4 are each as have been defined above, or their reactive derivative in the amino group or a salt thereof, with a compound of the formula-03: ** (Formula-03) ** where R1 and R2 are each as defined above or its reactive derivative at the carboxy group, or a salt thereof and, in case necessary, subjecting the resulting compound, where R1 is a protected amino, to an elimination reaction of the amino protecting group, to give a compound, where R1 is amino, and further, if necessary, subjecting the resulting compound, where R2 is a carboxy-lower alcohol protected to a carboxy-protecting group elimination reaction to give a compound, where R 2 is a carboxy-lower alcohol.
