abstract A method for diagnosing 1st insulin-like growth factor 1 deficiency (1st IGFD) or 2nd IGFD, said method comprising the steps of: a) determining a first concentration of IGF-1 and a first concentration of IGFBP3 in blood obtained from a patient and calculation of IGF-1 production rate in blood at baseline to provide a first IGF-1 production rate standard deviation score (IGF-1 PR SDS) , where the IGF-1 production rate is calculated using the algorithm: IGF-1 production rate = (IGF-1 blood conc.) (IGF-1 clearance rate), where the first IGF-1 PR SDS is calculated using the algorithm: IGF-1 PR SDS = (xp - mean age) ÷ SDage, where x is the rate of IGF-1 production in the blood, and p is the empirically determined energy to which the IGF-1 concentration to reduce skewness and kurtosis as close as possible to zero, where SDage is a value derived from a mean smooth curve generated by plotting IGF-1 blood concentration values as a function of age, where meanage is the mean age on the mean smooth curve used to determine SDage; wherein the clearance rate of IGF-1 is determined based on the first concentration of IGFBP3; b) determination of a second concentration of IGF-1 and a second concentration of IGFBP3 in blood obtained from said patient after the administration of GH in an amount sufficient to stimulate the production of IGF-1, and calculating the rate of IGF production -1 in blood to provide a second IGF-1 production rate standard deviation score (IGF-1 PR SDS), in which the IGF-1 production rate is calculated using the algorithm: IGF-1 production rate = (IGF-1 blood conc.) (IGF-1 clearance rate), where the second IGF-1 PR SDS is calculated using the algorithm: IGF-1 PR SDS = (xp - mean age) ÷ SDage, where x is the rate of IGF-1 production in the blood, and p is the empirically determined energy to which the IGF-1 concentration must be raised to reduce skewness and kurtosis as close as possible to zero, at that SDage is a value obtained from a smooth mean curve generated by plotting the blood concentration values of IGF-1 as a function of age, where meanage is the mean age on the mean smooth curve used to determine SDage; wherein the clearance rate of IGF-1 is determined based on the second concentration of IGFBP3; c) calculating a change in IGF-1 PR SDS between said first and second IGF-1 PR SDS (ΔIGF-1 PR SDS); and d) diagnosis - 1st IGFD if ΔIGF-1 PR SDS is less than +0.5; - a mixture of 1st IGFD and 2nd IGFD if ΔIGF-1 PR SDS is between 0.5 and +1.5; or - 2nd IGFD if ΔIGF-1 PR SDS at least +1.5.
