abstract | A high speed spin atherectomy system (100) for delivery of at least one therapeutic agent (115) into a biological lumen wall during a high speed spin atherectomy procedure within the biological lumen, comprising: a cable guidewire (15) having a maximum diameter less than the diameter of the biological lumen; a rotatable, elongate, flexible drive shaft (20) capable of advancing on the guidewire (15), wherein the drive shaft (20) has an axis of rotation (21); an eccentric abrasion head (28a, 28b) comprising an external surface and attached to the drive shaft (20), wherein the eccentric abrasion head (28a, 28b) comprises a center of mass that is offset from the axis of drive rotation (21) of the drive shaft (20); a therapeutic agent reservoir (18); a fluid delivery line in fluid communication with the therapeutic agent reservoir (18); a pump in operative connection with the therapeutic agent reservoir (18); a controller (19) in operative communication with the pump and therapeutic agent reservoir (18); a non-rotating therapeutic agent delivery sheath (22) comprising a sheath lumen therethrough, wherein the drive shaft (20) is rotatably slidably positioned in the sheath lumen; a catheter (13) disposed over the non-rotating therapeutic agent delivery sheath (22); and means for motorically urging the at least one therapeutic agent (115) radially outwardly away from the eccentric abrasion head (28a, 28b) and toward the wall of the lumen; characterized in that the non-rotating therapeutic agent delivery sheath (22) further comprises an inflatable balloon (205) disposed at a distal end thereof, wherein the lumen of the sheath is in fluid communication with the therapeutic agent reservoir (18). ) and inflatable balloon (205), and inflatable balloon (205) comprises a plurality of pores (210) in direct fluid communication with the lumen of the sheath. |
