abstract | An automated process for the preparation of a sterile radiopharmaceutical composition labeled with 123I comprising a molecule directed to a biological target labeled with 123I in a biocompatible carrier medium, wherein said method comprises: (i) providing a sterile single-use cassette comprising a reaction vessel and different aliquots of the following non-radioactive reagents (A) and (B) and optionally (C) to (G), each in sterile form: A. the molecule directed to a biological target or a precursor Of the same; B. suitable solvent (s) for the dissolution of reagent A and reagents C to G if present, including at least one solvent that is a biocompatible transport medium; C. an oxidation agent capable of oxidizing the iodide ion to a kind of electrophilic iodine; D. a source of iodide 127I; E. a non-radioactive, bifunctional derivatization reagent capable of conjugating with the molecule directed to a biological objective; F. a termination reagent capable of reducing said species of iodide ion electrophilic iodine; G. a catalyst for nucleophilic halogenation reactions; (ii) provide a sterile source of iodide 123I in a suitable container; (iii) radioiod the molecule directed to a biological objective through steps (iv), (v) or (vi); (iv) transfer reagent A and an aliquot of iodide 123I of (ii) to said reaction vessel microprocessor-controlled manner, optionally in the presence of reagent C, followed by mixing therein to obtain the molecule directed to a marked biological target with 123I, optionally using reagent F to stop said radioiodination reaction; or alternatively (v) radioiod the bifunctional derivatization reagent by microprocessor-controlled transfer to said reagent vessel of reagent E and an aliquot of iodide 123I of (ii), optionally in the presence of reagent C, followed by mixing therein to obtaining the 123I-labeled bifunctional derivatization reagent, optionally using reagent F to stop said radioiodination reaction, followed by radioiodination of the molecule directed to a biological target by conjugation with said 123I-labeled bifunctional derivatization reagent; or alternatively (vi) when reagent A is suitable for halogen exchange reactions, transfer reagent A and an aliquot of iodide 123I of (ii) to said reaction vessel microprocessor-controlled, optionally in the presence of reagent G, followed by mixing therein providing the molecule directed to a biological target labeled with 123I, optionally using heat to accelerate said radioiodinated reaction; (vii) when the molecule directed to a biological target marked with 123I product of steps (iv) to (vi) is already in a biocompatible transport medium, it is used directly in step (viii), if not, the product of the steps (iv) to (vi) either dissolve in a biocompatible transport medium or the solvent used in steps (iv) to (vi) is removed and the residue is re-dissolved in a biocompatible transport medium; (viii) the product of step (vii) is either used directly or optionally subjected to one or more of the following additional processes: purification; pH adjustment; dilution; concentration or final sterilization, providing the radiopharmaceutical composition with 123I. |
