abstract |
The aim of the invention is technical measures for producing a fistula to Schlemm's canal, with the aim of minimizing the scarring stimulus in order to maximize the duration in which the fistula can fulfill its purpose. This is achieved by puncturing the Schlemm's canal internally with a Q-switched erbium laser that has a pulse length of well under 200 ns. Preoperative filling of the Schlemm's canal with a biocompatible fluorochrome, which can be excited with the pilot laser, serves to avoid complete perforation. The fluorescence detected via the active waveguide serves as a switch-off criterion for the active radiation, so that only Schlemm's channel is opened. According to the invention, the scarring stimulus is further reduced by the action of the fluorochrome as a photosensitizer for producing atraumatic cell necrosis. |
