abstract Systems and methods are described for monitoring patients with various chronic diseases. The system may include a health status monitor that receives diagnostic data including physiological signals sensed from the patient. The system can generate at least a first indication of risk for a patient to develop a first disease and a second indication of risk for a patient to develop a second disease that is different. The system can detect the first and second diseases from the physiological signals and generate a composite health status indicator using the detection of the first and second diseases and the first and second risk indicators. If the composite detection score exceeds an alert threshold, an alert for a deteriorating health state may be generated.
